I understand that I'm opting for an elective procedure that is not urgent or medically necessary.

 

I also understand that Covid-19 has been declared a worldwide pandemic by the World Health Organization. I further understand that Covid-19 is contagious and can spread by person-to-person contact as where Federal and State Health agencies have recommended social distancing. Mindy Skincare has put in additional preventative measures in place to prevent the spread. However, Mindy Skincare services require person-to-person contact. I assume the risk of becoming infected with Covid-19 by receiving this elective procedure and give permission to Mindy Skincare and its staff to act accordingly through person-to-person contact in order to receive my procedure properly.

 

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself including but not limited to: personal injury, disability, death, illness, damage, loss, claim, liability or expense of any kind (hereinafter “Claims”) that I may experience or incur in connection with Mindy Skincare. On my behalf, I hereby release covenant to not sue, discharge, or blame Mindy Skincare, its employees and any representatives and from the claims, including all liabilities, claims, actions, damages, cost or expenses are rising out of or relating to. I understand that this release includes any claims based on the actions, emissions, or negligence of Mindy Skincare, its employees and any representatives related to the business.
